CHADIS Pre-Visit Questionnaires

Our office uses CHADIS, a web-based screening, diagnostic and management system, to administer pre-visit online questionnaires completed by parents, guardians,s, and patients.

This provides us instant access to valuable clinical data about your child's health and development, which helps us quickly determine the best treatment plan for your child. These questionnaires are integral to well-child and behavioral health visits and allow us to provide the highest quality of care.

They are available through your patient portal, and you will receive an email notification when you have a form to complete. I would need to complete these before your child's scheduled appointment.

For children ages 12 years and up, you may receive a parent and child questionnaire completed.

Most insurance companies cover CHADIS questionnaires; however, verifying coverage with your insurance company requires effort. Questionnaires that are denied by insurance will be the responsibility of the patient.
