CHADIS PRE-VISIT QUESTIONNAIRES
Our office will be using CHADIS which is a web-based screening, diagnostic and management system to administer pre-visit online questionnaires competed by parents and patients.
This provides us with instant access to valuable clinical data about your child's health and development which helps us quickly determine the best treatment plan for your child. These questionnaires are an integral part of well child and behavioral health visits, and help us to provide the highest quality of care.
They will be sent to you through your patient portal and you will receive an email notification when you have a form to complete. These need to be completed PRIOR to your child’s scheduled appointment.
For children ages 12 years and up you may receive both a parent and child questionnaire to be completed.
Most insurance companies cover CHADIS questionnaires, however, it is your responsibility to verify coverage with your insurance company.
