Transfering In?

Welcome!! We are excited to have you join our Pediatric Dream Care Team. Please read our website to determine if we are a good fit, and then just CALL OR EMAIL US at pediatricdreamcare@gmail.com. You can also request a "Meet the Doc" appointment at first, if you'd like to meet us.

ALL WE NEED TO GET STARTED :

We do require all forms to be completed before we can schedule your first appointment. 

1. New patient registration forms (one for family) AND patient history form (one per child)
2. Immunization records, and other significant medical records.
3. Copy of your insurance card.

Transfering Out?

We are sad to see you go. We know there are many reasons people have to leave at times and its helpful for us to know. Please let us know whether its due to age, insurance changes or just not a right fit. We strive to do our best and welcome feedback.

Patients tranfering out of our practice (due to aging out or transfering practices) will need to fill out a Medical Release Form
