Becoming a New Patient
It is crucial when choosing a pediatric practice for your children that you have given serious consideration to the fit between the practice philosophies and your own.

How Does My Child Become a Patient?
For Newborns
In the hospital, your baby will be examined by one of the very experienced newborn hospitalist physicians. As soon as you know when you are being discharged from the hospital, please call our office. We will schedule your baby’s first visit within 1-3 days after discharge.
The doctor will determine when your first appointment should be scheduled. Upon discharge, call our office to schedule your baby's first appointment.
For New Families Transferring to Us
Our goal is to provide the best medical care for your children. To do this successfully, we must be able to review all of your children's medical records from all providers that your children have seen.
If you are interested in transferring your child from another office, please call, and we will review the transfer process with you. We will provide you a packet of information for you to complete and sign a records-release request to receive previous medical and immunization records.

Once the records are received, we will review them to determine if they are complete or sufficient for us to be caring for your children.

If you would like to arrange for a tour of the office or to meet with Dr. Garofalo first, please let us know.
Please be aware that under federal (HIPAA) and state laws, your previous medical provider may charge a "reasonable, cost-based fee" to copy your childrens medical records. This cost is your responsibility. This fee may be waived if records are sent directly to our office.
